Emergence of artemisinin-resistance on Thai-Myanmar border raises spectre of untreatable malaria

ScienceDaily (Apr. 8, 2012) ? Evidence that the most deadly species of malaria parasite, Plasmodium falciparum, is becoming resistant to the front line treatment for malaria on the border of Thailand and Myanmar was reported in The Lancet April 5.This increases concern that resistance could now spread to India and then Africa as resistance to other antimalarial drugs has done before. Eliminating malaria might then prove impossible.

The study coincides with research recently published in Science in which researchers in south east Asia and the USA identify a major region of the malaria parasite genome associated with artemisinin resistance. This region, which includes several potential candidate genes for resistance, may provide researchers with a tool for mapping resistance.

Both studies, funded by the Wellcome Trust and the National Institutes of Health, follow reports in 2009 of the emergence of artemisinin-resistant malaria parasites in western Cambodia, 800km away from the Thailand-Myanmar border where the new cases of resistance have been observed. Resistance to artemisinin makes the drugs less effective and could eventually render them obsolete, putting millions of lives at risk.

According to the World Malaria Report 2011, malaria killed an estimated 655,000 people in 2010, mainly young children and pregnant women. It is caused by parasites that are injected into the bloodstream by infected mosquitoes. Plasmodium falciparum is responsible for nine out of ten deaths from malaria.

The most effective antimalarial drug is artemisinin; the artemisinin derivatives, most commonly artesunate, have the advantage over other antimalarial drugs such as chloroquine and mefloquine, of acting more rapidly and having fewer side-effects and, until recently, malaria parasites have shown no resistance against them. Although the drugs can be used on their own as a monotherapy, and these can still be obtained, fears over the possible development of resistance led to recommendations that they should only be used in conjunction with one or more other drugs as artemisinin-based combination therapies (ACTs). These are now recommended by the World Health Organization as the first-line treatment for uncomplicated falciparum malaria in all endemic countries. ACTs have contributed substantially to the recent decline in malaria cases in most tropical endemic regions.

In the Lancet study, researchers at the Shoklo Malaria Research Unit on the border of Thailand and Myanmar, part of the Wellcome Trust-Mahidol University-Oxford University Tropical Medicine Research Programme, measured the time taken to clear parasites from the blood stream in 3,202 patients with falciparum malaria using oral artesunate-containing medications over a ten year period between 2001 and 2010.

Over this period, the average time taken to reduce the number of parasites in the blood by a half -- known as the 'parasite clearance half-life' -- increased from 2.6 hours in 2001 to 3.7 hours in 2010, a clear sign that the drugs were becoming less effective. The proportion of slow-clearing infections -- defined as a half-life of over 6.2 hours -- increased over this same period from six to 200 out of every 1000 infections.

By examining the genetic make-up of the parasites, the researchers were able to provide compelling evidence that the decline in the parasite clearance rates was due to genetic changes in the parasites which had made them resistant to the drugs.

This finding is supported by the evidence reported in Science, in which the same researchers, together with an international team led by scientists at the Texas Biomedical Research Institute, San Antonio, identified a region on chromosome 13 of genome of the P. falciparum parasite that shows a strong association with slow parasite clearance rates. Whilst the actual mechanism involved is not clear, the region contains several candidate genes that may confer artemisinin resistance to the parasite.

Professor Fran?ois Nosten, Director of the Shoklo Malaria Research Unit, said: "We have now seen the emergence of malaria resistant to our best drugs, and these resistant parasites are not confined to western Cambodia. This is very worrying indeed and suggests that we are in a race against time to control malaria in these regions before drug resistance worsens and develops and spreads further. The effect of that happening could be devastating. Malaria already kills hundreds of thousands of people a year -- if our drugs become ineffective, this figure will rise dramatically."

Professor Nick White, Chairman of the Wellcome Trust's South-East Asia Major Overseas Programmes and Chair of the WorldWide Antimalarial Resistance Network (WWARN), added: "Initially we hoped we might prevent this serious problem spreading by trying to eliminate all P. falciparum from western Cambodia. Whilst this could still be beneficial, this new study suggests that containing the spread of resistance is going to be even more challenging and difficult than we had first feared."

Dr Tim Anderson from the Texas Biomedical Research Institute, who led the genetics studies in both papers, commented: "Mapping the geographical spread of resistance can be particularly challenging using existing clinical and parasitology tools. If we can identify the genetic determinants of artemisinin resistance, we should be able to confirm potential cases of resistance more rapidly. This could be critically important for limiting further spread of resistance.

"We know that the genome region identified harbours a number of potential genes to explore further to see which ones drive artemisinin resistance. If we can pinpoint the precise gene or genes, we can begin to understand how resistance occurs."

The Wellcome Trust-Mahidol University-Oxford Tropical Medicine Research Programme is one of the Wellcome Trust's major overseas programmes, working to achieve the Trust's strategic priorities, which include combating infectious diseases.

Dr Jimmy Whitworth, Head of International Activities at the Wellcome Trust, said: "These two studies highlight the importance of being vigilant against the emergence of drug resistance. Researchers will need to monitor these outbreaks and follow them closely to make sure they are not spreading. Preventing the spread of artemisinin resistance to other regions is imperative, but as we can see here, it is going to be increasingly difficult. It will require the full force of the scientific and clinical communities, working together with health policymakers."

Presage Looks to Help Millennium Pick Cancer Drug Winners Early ...

Seattle-based Presage Biosciences, a?spinoff from the Fred Hutchinson Cancer Research Center, is looking to help cancer drugmakers separate the winning compounds from the losers early in development, and now it?s found a?major partner to put this idea to the?test.

Cambridge, MA-based Millennium, a?unit of Takeda Pharmaceuticals, said today it has struck an agreement to gain access to Presage?s technology for analyzing how various combinations of cancer drugs work against human tumor samples. Financial terms aren?t being disclosed, but Presage will get upfront fees and research funding as well as development and regulatory milestone payments once Millennium selects successful drug combinations, the companies said in a?statement.

Presage, founded with technology from the lab of Jim Olson at the Hutch, has raised $4 million in its Series A?financing, and has formed collaborations with several Big Pharma companies that have tested its technology. Those partners are hoping to improve the odds of success in clinical trials, where only about one out of every 10 cancer drugs that enters clinical trials ever makes it through the hoops necessary to become an FDA-approved product. Millennium, the cancer drug development unit within Takeda, is best known for its blockbuster for multiple myeloma, bortezomib (Velcade), and it is now charged with building on that momentum to make Takeda a?bigger player in cancer drug development.

?It couldn?t be more exciting. We?ve talked to a?lot of companies, and we look to those with a?true innovative spirit,? says Presage founder Jim Olson. ?It?s uncommon to find groups with the track record and a?true innovative spirit, and Millennium is one of those?companies.?

While companies have long used various mouse models and screening technologies to decide which compounds to move into clinical trials, Presage has an unusual approach in which it directly probes human tumor samples. The company?s device uses an array of needles that have holes along the sides that can deliver multiple different kinds of chemotherapy drugs?or combinations of experimental biotech treatments?to localized regions of the tumor. The drugs can seep out within a?small radius of the needle, so researchers can see how different regions of the same tumor respond to different drugs in their native microenvironment.

One of the operative words in cancer research today is ?combinations,? which Olson said people are buzzing about this week at the American Association for Cancer Research meeting in Chicago. Companies are becoming more interested in finding those instances in which a?single drug doesn?t work on its own, but can show a?potent effect when given synergistically with another compound that works in a?different way.

?One of Millennium?s goals is to advance the treatment of cancer by developing novel therapeutic combinations,? said Joseph Bolen, Millennium?s chief scientific officer, said in a?statement. ?Presage?s technology platform allows us to obtain in-depth non-clinical in vivo data that is typically difficult to gather by conventional approaches. It is our hope that the Presage platform will help us to more rapidly and accurately prioritize our development of novel combinations.?

Presage is limited by its contracts in what it can say about its previous collaborations, but Olson did say partners have used its technology to make decisions on which drug candidates to move forward in development, and which ones to cancel?decisions that could have saved tens of millions of dollars in some?cases.

Presage currently has 14 employees at its South Lake Union offices. The company isn?t saying which other drug companies have tested its technology, but Olson says the Millennium deal should lay the groundwork for a?maximum of three such collaborations. He wouldn?t say if the Millennium deal enables Presage to reach cash-flow break even or profitability, but said it?s a ?transformative? partnership that has given the company confidence to expand its office lease and prepare to hire more employees. The plan over the next several years is to grow to about 40 employees, Olson?says.

Young Americans less likely to drive

WASHINGTON (AP) ? Driving is becoming so last century.

Since the end of World War II, getting a driver's license has been a rite of passage for teens, but that's less and less the case. The share of people in their teens, 20s and 30s with driver's licenses has dropped significantly over the past three decades, not only the United States, but also in some other wealthy nations with a high proportion of Internet users, transportation researchers have found.

One possible explanation: Virtual contact through the Internet and other electronic means is reducing the need for face-to-face visits among young people, researchers say.

From 1983 to 2008, the share of 16- to 39-year-olds with driver's licenses declined markedly, with the greatest decreases among drivers in their late teens and early 20s, according to a study at the University of Michigan Transportation Research Institute in Ann Arbor. About 69 percent of 17-year-olds had a driver's license in 1983. By 2008, that had dropped to 50 percent. Among Americans ages 20 to 24 in 1983, nearly 92 percent had driver's licenses. Twenty-five years later, it was 82 percent.

The older the age group, the less dramatic the declines, the Michigan study found. But even among 35- to 39-year-olds, there was a 3.2 percent decline in the share of licensed drivers.

More recent data from the Federal Highway Administration indicates the trend has continued, according to a report released Thursday by the Frontier Group, an environmental organization, and the consumer-oriented U.S. PIRG Education Fund. The share of 20- to 34-year-olds without a driver's license decreased from 89.6 percent in 2000 to 84.3 percent in 2010, the report said.

Michael Sivak, co-author of the Michigan studies, also confirmed the continuing decline.

There are likely several factors behind the trend: a difficult economy for young workers, the high cost of buying and maintaining a car and a migration of young adults toward large cities, where there are more alternatives to driving, Sivak said. There also is evidence that social networking may be reducing the need to for face-to-face contacts, he said.

In countries where "more people use the Internet, there is a lower proportion of drivers," Sivak said.

A recent study in 14 countries found seven of them had experienced a similar decrease in the share of young people with driver's licenses: Sweden, Norway, Great Britain, Canada, Japan, South Korea and Germany.

Compared to the other countries that didn't experience a decline, the study found four characteristics common to the countries where young people are driving less: They are wealthier, a high share of the population is older, a high share of the population lives in very large cities and a high share of the population uses the Internet.

There isn't enough information to say for certain that teens and younger adults are replacing trips with social networking and other Internet usage, but "there is strong data supporting this hypothesis," Sivak said. The institute's research is paid for with federal, state and auto industry contributions.

Carmakers have been aware of the trend for about five years, said Gloria Bergquist, vice president of the Alliance of Automobile Manufacturers. It's one reason the industry is looking toward markets in Asia for growth, rather than in the U.S. and Europe, she said.

"For generations, the automobile has typified freedom," Bergquist said. "At 16, many people wanted to get their driver's license because that was the way people connected with their friends." Now, she said, "We're seeing people connect through their iPhones. That's their primary motivation ? they want to be in touch with their friends, so they are less focused on buying a vehicle."

Not surprisingly, the number of miles young Americans drive every year has also been declining as measured by the Transportation Department's National Household Travel Survey. From 2001 to 2009, the annual number of vehicle miles traveled by drivers 16 to 34 years old decreased from 10,300 miles to 7,900 miles per capita ? a drop of 23 percent, Thursday's report said.
